Candidates seeking admission to the 1st Year B.Pharm program must produce the following original documents at the time of admission:

SSLC / 10th Standard Marks Card (Original + 3 Xerox Copies for Date of Birth verification)
12th Standard / 2nd PUC Marks Card (Physics, Chemistry & Biology/Mathematics - Min 45% aggregate)
Transfer Certificate (TC) issued by the college/school last attended
Conduct / Character Certificate from the Head of the Institution last studied
Migration Certificate (Mandatory for Non-Karnataka, CBSE, ICSE and other board students)
Caste & Income Certificate (For SC / ST / Cat-1 / OBC Karnataka reservation applicants)
Study / Domicile Certificate (Minimum 7 years study in Karnataka for Government quota seats)
KCET / GMU Allotment Order & Fee Receipt (Applicable for quota admissions)
Aadhaar Card Copy (Self-attested copies of Student and Parent)
Recent Passport Size Photographs (6 color photos with white background + 2 stamp size)

D.Pharm (Diploma in Pharmacy)

Crucial Guidelines for Certificate Submission
Keep Multiple Copies: All original certificates are retained by the college and submitted to the University/Council for admission approval. Students are strictly advised to retain at least 10 attested photocopies and digital scanned PDFs before depositing originals.
Anti-Ragging Undertaking: Every student and their parent must submit the mandatory online Anti-Ragging Undertaking on the official portal (www.antiragging.in) and provide the reference number receipt.
Fee Receipt: Preserve the original admission confirmation fee receipt issued by the GMIPSR Accounts Office safely for all future verification.
